Admissions Office Documents Step 1. Go to the following webpage: www.unm.edu/grad and
click on “Apply Now.” Select and fill out either the online or paper application provided.
Application deadlines are available by clicking on the link located in the center of the page. The
Application Fee is $50.00.
Send these materials to the Admissions Office:
Submit one (1) official transcript from EACH academic institution you have attended to the
Admissions Office address which is given on the second page of the website.

3 Letters of Recommendation – It is recommended that you use the printable forms located on
the same OGS website (www.unm.edu/grad). On the home page click on OGS forms. The“Letter
of Recommendation Form” is located in the Admission Forms section-third entry. At least one
letter should be from an individual who can specifically address your academic background and
potential for graduate study. It is strongly recommended that your references give you back their
letters in a sealed envelope with their signatures over the seal. You can then submit all 3 letters

Welcome to your University of Louisville Physicians Family practice! We
are offering patient-centered medical care and are enthusiastic about our
relationships with our patients. In order to better serve your needs, we are
enclosing several forms and ask that you completely fill each form out.
The first sheet will help us learn more about you; please completely fill out this
form about your family history. The next sheet is titled, "Authorization for the
use and/or Disclosure of Protected Health Information", and you will need to
completely fill that out for our doctors to treat you to the best of their ability; it
gives us permission to review your medical records from your previous primary
medical facilities.
Following, please completely fill out the Registration, Social Services & Consent
Form. Next, you will find our Privacy Notice, followed by an acknowledgement that
you have received and understand our Privacy Policies. Finally, the last form is the
Office Acknowledgements and Policies form. Please read carefully and sign
your name at the bottom of the letter.
Please make sure to bring all of these forms with you to your first office visit.
Do not mail them back to the office. Also, please remember to always
bring your picture ID, current insurance cards and your co-payment. If your
health insurance requires you to select a primary care doctor please do so prior to
your office visit. Please bring in any and all medication you take, in their
original bottles, to your appointment.
If the patient is under 18 years of age he or she must be accompanied by an
adult and will need to bring a copy of their current immunization certificate.
Please arrive 15 minutes ahead of your scheduled appointment time so that if
you have questions about these forms or we need more information, we can

Dear Prospective Student:
Thank you for your interest in Black Hawk College! This packet contains everything you
need to apply for admission, to apply for the form 1-20 to apply to Black Hawk College's
scholarship for international students, and much more.
You must submit the Application for Admission, high school transcripts (translated in
English), and financial statements to the office of the International Student Program. Please
submit the required $20.00 application fee with all your documentation. The forms are
included in this packet. Specific regulations regarding full-time enrollment and related
issues are mandated for F1 visa students.
After we have received the required documentation and accept you as a student, we
will then send you all the paperwork that you must take along with your 1-20 to your embassy
or consulate to apply for a visa to enter the United States. Obtaining a visa can take several
weeks, so complete your paperwork as early as possible.
When you arrive at Black Hawk College, you will take the Michigan Test of English
Language Proficiency, which all foreign students must take. The TOEFL Exam is required.
You will also participate in an orientation program that guides you through important steps

5) Enter your La Verne student ID# in the student ID field. If you do not yet have your La
Verne ID please enter your date of birth in MM/DD/YYYY format.
6) Follow the instructions to enter your address and phone number. Click Next.
7) We recommend that you select “FED EX” as your carrier for faster service. Though FedEx
services most locations, you may choose to have your items mailed via DHL.
8) Enter your credit card information.
Errors in the credit card information or address information will result in a delay. A waybill will
be sent to our office and will be used to mail your shipment. Make sure that your address is
correct and deliverable. You will be able to track when your item has been mailed by logging
back into the EShipGlobal system.

Dear Prospective International Student:
Thank you for your interest in our University. An International Student Packet is enclosed for your use in
gaining admission to our school. We welcome international students to our campus and feel that we offer
outstanding opportunities for career training in more than eighty majors. Our catalog is available online at:
www.uvu.edu.
To apply at our University you will need to submit the following documents:
1. The Application (must be 18 years or older for admittance)
2. The "Affidavit of Support" form, plus a bank statement which must show 26,400.00 for
one academic year (8 months). This form must be filled out by a parent or sponsor and
submitted with an original bank statement or a tax return. This Affidavit and supporting
documents are required. Or, if you are paying your own fees, you may submit an original
bank statement showing at least 26,400.00 in U.S. funds, on deposit, under your name. Be
sure all funds are converted into U.S. dollars. These documents must be original. Your
sponsor will need to read and initial the information sheet provided in this packet.
3. Copy of your passport and visa (if you already have a visa).
4. Transcripts showing you have completed our level of high school.
5. The 100 non-refundable application fee in U.S. funds. Any collection or exchange fee is
your responsibility.
6. An International TOEFL or IELTS score (not necessary if you are applying for English as a
Second Language). See enclosed information sheet.
7. If you are transferring from another institution in the United States, you will need to have your
previous school fill out and send, directly to us, the enclosed TRANSFER CERTIFICATION
form. We will also need a copy of your current I-94 card.
Please note: The U.S. Department of Homeland Security now requires the completion of Form I901 and a fee remittance of 200.00 prior to your appointment for a Visa. This form and
instructions are included when the student I-20 is issued.
You must follow the application deadlines included in this packet. Allow three to four weeks for processing

Dear Prospective Family,
The Admissions Process includes completing our online student application which includes a $75
application fee, essay on why the student wants to attend Pope Francis Preparatory School,
choosing a Placement Test Registration date (8th graders only) and submitting a signed Information
Release Form. A personal interview is required for 8th graders and a meeting is required for all
transfer students to grades 10, 11, and 12 and required for all students seeking support per an IEP
or 504 Plan. Please note, students must complete all steps in this process before being eligible to
receive a letter of acceptance.
Open House for prospective students and their families will be held on Sunday, November 4,
2018. Program begins at 1:00 p.m. and registration is required.
The Placement Test for prospective ninth grade students who are interested in attending Pope
Francis Preparatory School in September 2019 will take place on Saturday, December 1, 2018, or
Saturday, February 2, 2019 from 9:00 to 11:00 a.m. at PFPS, 99 Wendover Road, Springfield, MA.
Decisions of acceptance are made based on the student’s overall middle school record (grades and
conduct), standardized test scores, the results of the High School Placement Test (HSPT), and
personal interview. Registration for the HSPT is included in the application for admission.
Shadow Days may be scheduled for a Tuesday, Wednesday, or Thursday between November 6,
2018 and May 16, 2019. A Visitor/Shadow form must be completed and submitted to the
Admissions Office at least one week prior to the visit. The form is available for download on the
Admissions page of our website at www.popefrancisprep.org. Students are not allowed to shadow
without first making arrangements with the Admissions Office. As always, visitors must be in
proper dress and must adhere to all Pope Francis Preparatory School rules and regulations. Please
see instructions on the Shadow Form.
